Storytelling Showcase Final Checklist "

*PLEASE SEND THIS CHECKLIST WITH YOUR ENTRY PACKET*

Before sending your VHS video entry to the Pigeon Forge Office of Special Events or your State Representative, be sure to make sure you have included the following items:

_____ 1. VHS video containing one (1) "National Youth
Storytelling Showcase" entry of approximately 5 to 10 minutes in length. Make sure your name and address are noted not only the cover, but the tape itself.

_____ 2. Completed Entry Form. (Use additional pages, if necessary).

_____ 3. Completed Story Permission Form - Required. (If subject is living.)

_____ 4. Completed Permission Form for Videotaping and Web Page usage - Required.

_____ 5. Completed Interview Release Form - Required, if story was developed from a personal interview.

_____ 6. Entrant Release Form - Required.

_____ 7. Verify that the mailing address is correct prior to mailing. It is as follows:
